Dear Parents and Guardians,
To support your student’s health and comfort during the school day, we are updating our procedure regarding over-the-counter (OTC) medications.
Previously, a doctor’s note was required for each OTC medication. Moving forward, we are introducing a streamlined OTC Authorization Form. This form allows our school health staff to administer common medications—such as ibuprofen, acetaminophen, or cough drops—to your child with written consent from both a parent/guardian and a licensed healthcare provider.
What you need to do:
Parent/Guardian Review & Signature: Complete and sign the Over-the-Counter Medication Authorization Form, indicating, along with your child's provider, which medications your child is permitted to receive.
Healthcare Provider Signature: Have your child’s physician or licensed care provider review, sign, and date the form.
Return the Form: Submit the completed form to your school health office.
Provide Medication (if needed): If your child requires a specific brand or liquid formulation, please provide an unopened, clearly labeled bottle with your child’s name.
Please note that the school will stock generic versions of the listed medications.
All OTC medication authorizations are valid for one school year and must be renewed annually. No medication will be administered without a current form on file.
Our goal is to minimize the time students spend out of the classroom while maintaining the highest standards of safety and care.
