Kindergarten Screening Canceled

Dear Parents,

Last Thursday, Governor Cuomo announced that all school districts in New York State will remain closed until May 15, 2020.  As a result, the Chatham Central School District’s 2020-2021 kindergarten registration dates have been cancelled. No plans to reschedule have been made.

Please be assured that we are still here and 100% committed to helping you get your child registered for this very exciting time in their lives.  We ask that you assist us by completing the forms that were sent in your child’s enrollment packet, gathering the needed documents and mailing them back to us in the stamped envelope that was provided.  Here is a quick checklist to assist you:

  • Student Registration Form

  • 2 recent Proofs of residency

  • Birth Certificate (or other official proof of birth)

  • Home Language Questionnaire

  • Student Pick-up Authorization and Dismissal Form (green)

  • Court Order involving Child Custody (if applicable)

  • Kindergarten Health History Questionnaire*

  • Certificate of physical examination from a physician*

  • Immunization record*

  • Dental Certificate*

The kindergarten teachers and I are saddened that the current situation has prohibited us from personally meeting you and your child at this time.  Unfortunately, until circumstances change, we are unable to schedule any future meet and greet.

During this unprecedented time we are aware that routine well-child visits may not be taking place as normal and that your child’s physical and age appropriate immunization appointments may be delayed. 

Please be assured that this will not delay the enrollment process or the start of school  for your child as long as:

  • Proof of all age appropriate immunizations are provided within 14 days of the start of school 

  •  a physical, or written proof from a physician of an appointment, is received within the first 30 days of school 

We understand you may not have an up to date physical at this time. This should not delay you sending in all of your registration paperwork. We request that you complete all health history questions as thoroughly as possible. If your child has any major medical concerns, please contact the school nurse, Mary-Noelle Fabiano RN,BSN at to set up a time to discuss the issues further.
