Authorization for Administration of Medication Form
Physical Education
Any student wishing to participate in extracurricular sports must have a current physical on file. Physicals are current for one year from the date of the form. If that physical is dated more than 30 days before the start of the sports season tryouts, the Athletic Health History Form must be filled out and returned to the nurse not more than 30 days before tryouts begin.
Working Papers
All students ages 14 - 17 who are interested in getting a job should fill out Part I of the Application for Employment Certificate form. Students should also check with the High School Nurse regarding their physical status; students must have had a physical within the last year.
