Here you will find information provided by Chatham's school nurses as we attempt to answer some of the common questions parents have. Additional information is posted on each school's individual Health Office website.
School nursing is a specialized practice of professional nursing that advances the well-being, academic success, and lifelong achievement and health of students.

- Health Appraisal Form (for physical done outside of school)
- Student Health History Form
Prior to entering school, children must be fully immunized. Documentation of immunization is required. For more information, call the health office in your child’s school building.
Any student wishing to participate in extracurricular sports must have a current physical on file. Physicals are current for one year from the date of the form. If that physical is dated more than 30 days before the start of the sports season tryouts, the Athletic Health History Form must be filled out and returned to the nurse not more than 30 days before tryouts begin.
All students ages 14 - 17 who are interested in getting a job should fill out Part I of the Application for Employment Certificate form. Students should also check with the High School Nurse regarding their physical status; students must have had a physical within the last year.
